Personal Care at Symphony at Valley Farms is grounded in compassionate care and rigorous attention to safety. Our care team will complete a health evaluation to develop a personalized care plan before move in, and on an ongoing basis as needs evolve.

As a licensed personal care community in Kentucky, we offer the following care and wellness services:

  • Dressing/Undressing Support

  • Assistance with Toileting Needs

  • Continence Management (not including supplies)

  • Assistance with Mobility Needs

  • Escorts To and From Meals and Events

  • Medication Management including the Ordering of Prescription Medications

  • Coordination with Outside Healthcare Providers

  • Mealtime Assistance

  • Status Checks

  • Weight and Vitals Monitoring (if physician ordered)
